I am currently recruiting RGN's B5 to work in the following location:

- North London

MAIN TASKS AND RESPONSIBILITIES Patient/customer care (both direct and indirect)

Effectively deliver skilled nursing care to patients using a systemic approach of assessing planning, implementing and evaluating to ensure a high standard of care.
Promote an exceptional standard of care in line with Essence of Care and Care Quality Commission standards and ensure that each patient has a care management plan that is effective and patient centred.

Assist in the admission and discharge processes for patients in line with local procedures.

Assist in effective patient admission pathways and start early and effective discharge planning. Use clinical skills to agree admissions and manage/report any impact on shifts/ workload and staffing levels.

To assist in the delivery of accurate, contemporaneous records/ care plans that ensure safety and continuity of patient care in a manner that reflects the NMC and Trust standards for Record Keeping and Data protection.

To assist in ensuring compliance with Health and Safety guidance and policies as directed by safety officers, ensuring that all staff operates within relevant legislation, including Codes of Practice.

Safeguard the welfare and safety of patients, staff and visitors to the Magnolia Unit by being aware and involved in the clinical risk assessment and guidelines for Magnolia Unit. To respond to action plans as directed by others or initiated by you. Recognise situations that may be detrimental to health and safety and act within the health and safety guidelines of Enfield Health.

To follow the Infection Control guidelines to ensure that the local and national Infection control guidelines are implemented and monitored through regular audit and review and any lapses in standards are reported appropriately.

Work closely with the Therapy staff, agreeing joint goals and outcome measures to promote independence and avoid unnecessary admission to acute and long-term care for patients in the Intermediate care beds.

To assist toward maintaining high standard of cleanliness in the Unit, ensuring standard to meet the requirements of Patient Environmental Action Team and Infection control by co-operating and reporting issues for attention are achieved.

Be responsive and supportive to the physical, social, psychological and spiritual wellbeing of each individual within the unit, positively contributing to team working.
Oversee and participate in the early identification/assessment of patients' needs in regard to Food and Nutrition and ensure patients receive adequate nutrition, including ordering, monitoring and documentation of same.

Provide, where appropriate, advice, guidance and training to patients/relatives, carers and other professionals involved in individual care, incorporating health promotion practice.

Be a recognised Mentor/assessor for peers/student nurses and others as requested and attend annual updates as indicated. Be involved in the orientation and teaching of new staff to the unit, including students and voluntary workers.
